Dental PDP/Indemnity - SHC420 (ValueChoice)

Use the Dental HMO plan directory to pre-select your general dentist if you are enrolled or wish to enroll in the DHMO plan . You and your enrolled dependents may each select different dentists.

You do not have to pre-select a dentist if you enroll in the Dental PDP/Indemnity (ValueChoice) plan. Just call a dentist listed in the PDP/Indemnity directory and let them know you are enrolled in the PDP/Indemnity plan for Harris County when making an appointment .
